GENERAL INSURANCE POLICY CONDITIONS
Here we summarize the insurance general conditions provided by the policy placed in Lombard Gate Srl office. These policy conditions, which may be transmitted on demand, are also fully available on www.lombardgate.it and www.golfholly.com websites.

1. Effective date – Expiry date – Operation
for "cancellation charges", insured capital equal to the travel cost,
performances and insurance guarantees start and become effective from travel booking and with the consequent contextual emission of the insurance guarantee and they decay with the fruition of the first service contractually agreed;
As for concerning "Interassistance 24 hours", insured capital up to € 1,000.00 (Italy) and € 10,000.00 (Abroad) and as for concerning "Luggage” insured capital up to € 500.00.
The performances and the insurance guarantees become effective and are valid for all people coming, from the fruition of the first service included in the travel contract and they end with the full accomplishment of the last formality included in the contract; for tourist, study and business trips, the coverage remains in the maximum limit of 60 days from the departure date;
2. Communication conditions
All the accidents covered by the insurance must be communicated to the Agency where the travel was booked not later than the first following working day.
Communications done by ensured people to Mondial Assistance, except for preventive calls to the Operations Centre, must be done in writing:
Anyway for “Interassistance 24 hours” please contact the Operations Call Centre, operating 24 hours, at the following numbers: phone 02 26609604 Fax 02 70630091. Please communicate a reachable telephone number and all the necessary intervention data, even the doctor's telephone number or, in case of hospitalization, the hospital/clinic data where the patient is and please send the original invoices concerning incurred expenditures together with diagnoses, prescriptions and, in case of hospitalization, the medical record. NB use, where possible, the health insurance covered by the National Healthcare Provider.
The Insured shall also, within 5 days, notify to Mondial Assistance Italy SpA, Damages Liquidation Service, Via Ampere, 30 - 20131 Milano - Fax +39 02 2665593 the events/accidents that occurred and its reimbursement request with the supporting documentation:
- For cancellation charges, renunciation or interruption of the vacation, the hospital medical records and the address where the sick or injured person is staying, or any other documentation which explains the reasons of the renunciation;
- For luggage, a copy of the claim about theft/loss/damage and a written complaint with the refund request to the responsible (air carriers, hotel manager, etc.), together with the invoices and/or tax receipts of incurred expenditures.
3. Taxes
The insurance package taxes must be paid by the Insured.
4. Not included (valid for all guarantees)
- any compensation, service, outcome and/or events directly or indirectly deriving from epidemics or quarantine with pandemic characteristics (declared by OMS), and with severity and virulence causing high mortality or requiring restrictive measures to reduce the risk of transmission to the civil population are excluded from insurance policy.
5. Privacy
According to Legislative Decree No. 196 of 6/30/2003 (c.d. Privacy) we inform that all the reserved information concerning the Insured will be exclusively used for the purpose of this particular insurance relationship.

CANCELLATION CHARGES
Object - The Insurance company will reimburse, within the insured capital, the cancellation fee (except for register fees) that the Insured must pay to the trip Organization or to the Carrier, as provided by the General Travel Contract Conditions if the booked trip or location cannot be started owing to the following documented reasons, unintentional and not foreseeable at the reservation time:
- Illness, accident or death of the Insured, his partner or live-in partner, children, brothers, sisters, parents in law, son or daughter in law;
- Insured Minors grandparents’ illness with hospitalization or death;
- In case of simultaneous registration of two people to the same trip, illness, accident or death of the simultaneously registered person and with whom the insured had to attend the trip, if insured, also for his partner, live-in partner, children, brothers, sisters, parents, parents-in -law, sons or daughters- in -law.
Insured brother or sister-in-law hospitalization injury or death.
- Illness, accident or death of the Insured Company or Office associate Member/Co-owner.
- Inability to take advantage of the planned accumulated holidays, owing to the Insured work dismissal /suspension (redundancy fund, mobility) or his new recruitment.
- Fire damages, burglary or natural disasters, affecting the Insured property or where the Insured develops his business, professional or industry activity, requiring his presence. Inability to reach the departure or lease place, residence place, owing to natural disasters at the residence place and declared not accessible by the competent authorities, or an accident to the means of transportation during the course to the departure place.
- Order notice to appear in court as a witness or communication to be part of a popular jury, if notified to the Insured after the booking date.
- Summoning to appear in court for children adopting procedures.
- Theft of the necessary expatriation documents, when it is proved the physical impossibility of their remaking before the scheduled departure date.
With reference to the previous points, it is clarified that, as for concerning diseases valid for cancellation, unpredictable relapses of diseases existing before the booking date are included, if not evolutionary or chronic. Pregnancy diseases are also included if the pregnancy started before the booking date.
In case of renunciation owed to illness, accident or death of any person specified in the first two paragraphs, and only if there are some minors among the Insured, the fines charged for the renunciation of any baby-sitter are also refund, if insured and expressly indicated on the booking confirmation.
Limitations – During the repayment, a discovered amount, representing 10% of the compensation amount, will be applied for every Insured with a minimum of € 50,00 in any case.
Exclusions:
- Accidents and pre-existing diseases before the booking date are not included in the insurance policy (except for the cases provided by the penultimate paragraph of Article. 1), as well as chronic, nervous, mental and psychosomatic diseases, except for those diseases due to alcohol abuse. Pregnancy is also excluded.
- Renunciations due to professional reasons are also always excluded, except for the impossibility to take advantage of the planned holidays for the reasons provided in the previous paragraph.
- If the Insured signs the travel contract with two or more non-related people, as part of a previously established group, or with other families, the renunciation of the person registering simultaneously is not considered a valid reason for the reimbursement. The refund will be considered only for those Insured directly involved.
- It being understood that, according to the other limitations of this Article, if the travel or the lease are cancelled later, Mondial Assistance reimburses the penalties existing at the moment of occurred event, if not higher than those actually applied (Article 1914 CC). Therefore, the higher charged penalty will be paid by the Insured.
In case of illness or injury of any person indicated in the previous articles, Mondial Assistance doctors can make a check up.
INTERASSISTANCE 24 hours
Object: Mondial Assistance, through its Operations Centre, active 24 hours, provides for furnishing the following services listed in the policy, within the limits indicated by the destination in the Insured Capital schedule and with a fixed exemption of € 25.00 paid by the Insured, including:
- Medical Advice;
- Sanity return of the Insured with the most appropriate means, and with medical/nursing assistance.
- Transfer of the corpse;
- Living, rescue and recovery expenditures, only after an accident, up to € 1,000.00 for travels abroad and up to € 500.00 for home travels.
- Return of an insured relative, up to a maximum of € 150.00 per person, maximum 2 people.
- Travel expenses for a relative if the Insured hospitalization exceeds 7 days.
- Return of under 15 years old children;
- Hospitalization amount, directly paid, up to € 200.00 per day (Abroad) and up to € 50.00 (Italy)
- Urgent and not extensible dental care costs, directly paid, up to 100.00 (Foreign) and up to € 50.00 (Italy)
- Care costs reimbursement up to € 10,000.00 (Abroad) and up to € 1.000.00 (Italy).
- Additional living costs up to € 100.00 (overnight stay only)
- Return to home of the convalescent insured, with the most appropriate means. If a medical transport is required, only medical needs will be taken into consideration in the choice of the transportation means.
- Early return due to a family mourning, up to the value of € 800.00 (Abroad), € 250.00 (Italy).
- Urgent medications sending.
- Urgent messages sending.
- Telephone expenses up to € 100.00, limited to abroad travels.
- Interpreter at your disposal up to € 500.00.
- Bail deposit only abroad up to € 2,500.00 per person, against the provision of adequate security.
- Advance money up to € 2,500.00, against the provision of adequate guarantee.
- Documents protection, in case of loss or theft.
Medical expense not included:
- Infirmity due to pathological conditions already existing at the departure, chronic diseases, mental or psychosomatic illnesses; pregnancy later than 180th day, accidents or illnesses due to alcohol and drugs that affect the mind, abuse and non-therapeutic use of drugs or hallucinogenic, immunodeficiency acquired syndrome (AIDS), to crimes committed or tried by the Insured, to suicide or attempted suicide, rehabilitative cares, nursing, physiotherapy, slimming and thermal cares, physical defects or congenital malformations removal cares, abortion;
- Purchase, maintenance or reparation of glasses, contact lenses, braces, physiotherapy and in general every kind of prosthesis;
- Aesthetic actions or applications;
- Inspections carried out after home return resulting from situations started during the vacation;
- All costs incurred by the Insured without the prior permission of the Operations Centre.
If the Insured does not take advantage of one or more performances, Mondial Assistance is not required to pay compensation or provide alternative services as compensation. If Mondial Assistance will arrange the return of the Insured, he is required to return the unused tickets.
- Illnesses or accidents occurred during the professional activity are also excluded, except for those having commercial nature.
Some events are also excluded, if caused by:
• travel undertaken against medical advice, with acute illnesses or in order to undergo treatment or medical tests or surgeries, organ removal and/or transplantation;
• driving a vehicle not for private use, and any vehicle or motor vessel, if the Insured does not have a prescribed qualification.
• nuclear explosions, radioactive contaminations, tornadoes, hurricanes, earthquakes, floods and volcanic eruptions, and any other natural or atmospheric phenomenon; war acts even if civil, strikes, riots, insurrections or popular demonstrations, curfews, borders block, retaliation, sabotage, terrorism, unless the Insured proves that the accident has no connection with these events;
• trainings, tests, car, motorcycle or motor boating races, caving, mountain climbing up to 3rd degree carried out in isolation, all climbing with a degree higher than 3rd, free climbing, rafting, bungee jumping, jumping with skis or water skiing, acrobatic skiing, mountaineering skiing, skydiving, hang gliding, paragliding and practice of air sports in general and any other dangerous sport. Accidents caused by participating in some competitions are excluded, except for those with a recreational purpose.
Vacation interruption:
Mondial Assistance refunds the pro-rata of the remaining days of vacation, as follows:
- In case of the Insured sanitary return, managed and carried out by the Operations Centre, as determined by Interassistance warranty contractual terms, as for concerning the "Sanitary return", the reimbursement is recognized only to the sick or injured Insured;
- In case of early return due to a relative mourning, planned by the Operations Centre, as determined by the Interassistance warranty contractual terms concerning "early return", reimbursement shall be granted to the early returned Insured.
The reimbursement covers only the living fee (excluding, therefore, airline/rail/sea tickets etc.., and the registration fees). For pro-rata calculating purposes, we precise that the day of interruption of the vacation and the day of the return are considered as a single day.
LUGGAGE
Object: This insurance is paid to first loss, up to the amount of € 500.00 and, therefore, Mondial Assistance will pay the damages without taking any account of the value of the luggage amount, it being understood that it can be subject to the lien to the damage responsible, according to Article. 1915 Cod. Civ.
All the things that the Insured takes with him for his personal needs during the journey are ensured against theft, fire, robbery, failure to return and/or damages by the Carrier, as well as the luggage shipped by transportation companies.

In case of air travel, it is reimbursable up to a maximum of € 100.00, with the insured limit of indemnity, purchases incurred abroad as a result of delays beyond 12 hours in luggage delivery, with the airline evidence, are refundable too. The request must be documented by original invoices or purchase receipts.
The insurance covers the total or partial loss and damage up to the insured amount. The indemnity for total or partial loss and the damage of the insured items is calculated on their market value at the moment of the accident. If the insured items have suffered from a depreciation caused by use, or for other reasons, this will be fairly taken into account. In case of damage the reparation amount will be refunded, upon invoice presentation.
In case of damages suffered at the Airport:
- Delivery failure or damage: do immediately the RIP (Property Irregularity Report) at "Lost and Found" airport office. Always forward a written complaint to the carrier;
- theft: regular written complaint to the airport police station.
In case of damages suffered in other circumstances:
write immediately a complaint to the local police authorities. Always forward a written complaint with the reimbursement request to the responsible Carrier or the Hotelier.
Limitations: Covered up to 50% of the insured cumulative capital and the maximum compensation for each object or good cannot exceed the amount of € 50.00:
a) jewels, pearls, gemstones, watches and gold , platinum or silver items, furs and other valuables: these items are insurance covered only if worn or carried on;
b) cameras, radios, televisions, tape recorders, electronic equipment: these items are not covered by insurance if included in the luggage delivered to a transport company or entrusted to third parties (hoteliers, restaurateurs, etc.).. Photo or camera kits (camera, television camera, binoculars, lenses, flash, filters, batteries, bags, etc...) are considered as a single object.
In case of third parties liability (carriers, hotels, etc...) Mondial Assistance compensation will happen after the Carrier or the hotelier compensation, net of the sum already compensated and only if the compensation above referred is smaller than the maximum insured.
Also excluded:
a) Money in all its forms (bills, checks, traveller’s checks, credit cards, etc..), travel tickets, documents, securities and all kinds of collections, art objects, weapons in general, goods, professional equipment, mobile phones, personal computers, samples, films, magnetic tapes and compact discs, camping equipment, sporting equipment in general and helmets;
b) Damages caused, directly or not, by Authorities’ displays, by war, civil or military disorders, riots, strikes, mines, pillages and earthquakes, unless the Insured proves that the incident has no connection to the event;
c) Damages intentionally caused or due to the Insured strong negligence;
d) Damages caused by normal use, inherent defects, fluid leakage, moths, worms, bad/inadequate or inappropriate packaging, weather;
e) Breaking damages, due to an accident involving transport means, external facts, burglary, robbery, assault with a deadly weapon, fire extinguishing or fire;
f) damages due to forgetfulness, mislay or loss of an object and fall of pearls or precious stones from their embedding;
g) Insured staff theft or damage;
h) theft of luggage left on motorcycles boards, mopeds and bicycles;
i) Car radios and music tapes, as well as fixed accessories and for service of any vehicle;
j) Goods bought during the trip.
k) luggage, objects left in the car, camper or caravan are insured only if placed in a place not visible from outside or in the locked luggage van, and if the vehicle is parked in a garage or in a supervised park which you have to pay to use.
Lombard Gate S.r.l. in collaboration with Borghini and Cossa Srl brokers and Mondial Assistance, an insurance company specialized in tourism coverage policies, agreed to specific insurance policy terms and favourable conditions, that participants are obliged to arrange at the booking time.
The insurance premium is not separated from the individual travel cost, since it is integral part of it and non-refundable.
The considered guarantees are “Cancellation charges”. "Interassistance 24 hours ", "Luggage" and their related conditions are also fully contained in the insurance certificate that will be delivered, with other travel documents, before the departure.
Insurance package agreement modality:
Each customer/traveller will pay, at the booking time; the insurance premium, according to the travel/living individual cost, as determined by the following table:

Double insurance
If the Insured benefits, through other companies or insurance companies, from services or coverage similar to those provided under Article. 1 of the Insurance Certificate, the Insured must expressly decide which Operations Centre will do the intervention.
